    Context:

    • Pacemaker syndrome (PS) is a clinical complication following pacemaker implantation.
    • VVI pacing, a common mode, can lead to adverse hemodynamic effects.

    Purpose:

    • To investigate the incidence and clinical manifestations of pacemaker syndrome in patients with VVI pacemakers.
    • To determine the association between retrograde ventriculoatrial (VA) conduction and the occurrence of PS.

    • 19 cases of PS were observed in 121 patients (15.7%) with VVI pacemakers, presenting with dizziness, hypotension, and fatigue.
    • PS incidence was higher in sick sinus syndrome (20%) than A-V block (13.2%).
    • Retrograde VA conduction was present in 25 patients, with 56% experiencing PS compared to 5.2% without VA conduction (P < 0.0001).
