Summary

Mandibular implant overdentures using 2 or 4 mini-implants improved oral health-related quality of life and satisfaction compared to standard implants. However, mini-implants showed lower survival rates.

Area of Science:

  • Dentistry
  • Oral Implantology
  • Prosthodontics

Background:

  • Mandibular implant-retained overdentures are a preferred treatment for edentulism.
  • Standard implants face limitations like ridge width, health issues, patient fear, and cost.
  • Mini-implants offer a potential alternative for edentulous patients facing these challenges.

Purpose of the Study:

  • To compare the efficacy of mandibular overdentures retained by 2 or 4 mini-implants versus standard implants.
  • To evaluate oral health-related quality of life (OHRQoL), patient satisfaction, and implant survival rates.

Main Methods:

  • A randomized trial involving 120 edentulous participants (mean age 59.5 years).
  • Participants received either 4 mini-implants, 2 mini-implants, or 2 standard implants.
  • Data on OHRQoL, satisfaction, and clinical parameters including implant survival were collected over 12 months.

Main Results:

  • Both 2 and 4 mini-implant groups reported significantly better OHRQoL than the standard implant group.
  • Patient satisfaction was highest with 4 mini-implants, intermediate with 2 mini-implants, and lowest with 2 standard implants.
  • Implant survival rates were 89% for 4 mini-implants, 82% for 2 mini-implants, and 99% for 2 standard implants.

Conclusions:

  • Mandibular overdentures supported by 2 or 4 mini-implants can provide OHRQoL and satisfaction comparable to standard implants.
  • While mini-implants offer benefits, their survival rate is lower than that of standard implants.
  • The choice between mini-implants and standard implants should consider patient-specific factors and the trade-off between OHRQoL and implant longevity.
