Summary

Sloped shoulder dental implants, immediately placed in the anterior maxilla, demonstrated high survival rates and preserved marginal bone and soft tissue aesthetics over three years, even with bone defects.

Background:

Purpose of the Study:

  • To evaluate the 3-year clinical outcomes of immediately placed and provisionalized OsseoSpeed™ Profile implants with sloped shoulders.
  • Assess survival and success rates, marginal bone levels, and Pink Esthetic Score (PES).

Main Methods:

  • 21 OsseoSpeed™ Profile implants were placed flaplessly into extraction sites in 16 patients.
  • Immediate provisionalization was performed; facial gaps were grafted with autogenous bone chips.
  • Clinical and radiographic evaluations included survival, success, bone levels, and PES.

Main Results:

  • After a mean 43-month follow-up, 19 of 21 implants (90.5%) remained functional.
  • Interproximal marginal bone loss averaged -0.2 ± 0.4 mm apical to the implant shoulder.
  • The mean Pink Esthetic Score (PES) was 11.9 ± 1.4, indicating favorable soft tissue aesthetics.

Conclusions:

  • Sloped shoulder implants, when immediately inserted and provisionalized, can maintain marginal bone and soft tissue aesthetics.
  • This approach is effective even in cases with pre-existing facial bony wall defects.
  • The study supports the use of sloped implants for predictable outcomes in the anterior maxilla.
