Background:
- The aortic wall relies on adventitial vasa vasorum (VV) for perfusion.
- Tissue hypoxia is a known feature of enlarged abdominal aortic aneurysms (AAAs).

- To investigate if hypoperfusion of adventitial VV can cause abdominal aortic aneurysms (AAAs).

- A novel rat model was developed using catheter insertion and aortic ligation to induce VV hypoperfusion.
- Morphological and pathological characteristics of the induced AAA were compared to human AAA.

- Human AAA tissue, regardless of size, showed stenotic adventitial VV, with ischemic and hypoxic sac tissue.

- Hypoperfusion of adventitial VV is a critical factor in the development of infrarenal abdominal aortic aneurysms.
- Findings suggest a direct link between compromised VV blood supply and AAA pathogenesis.
