Background:

Spinal hamartoma is an exceptionally rare, benign spinal tumor.
It is most commonly diagnosed in pediatric patients with spinal dysraphism or neurofibromatosis type 1.

Observation:

This report details a unique case of a thoracic spinal hamartoma in a 75-year-old male patient.
This individual is the oldest documented case among 19 detailed reports and one of only nine reported cases lacking associated lesions.
The lesion presented as a well-defined, exophytic mass originating from the dorsal midline of the spinal cord.

Findings:

Magnetic resonance imaging (MRI) revealed a distinct exophytic lesion on the dorsal midline of the spinal cord.
The case contributes to the limited understanding of spinal hamartoma in adult populations.
Radiological and pathological features were analyzed in the context of existing literature.

Implications:

This case expands the known age range for spinal hamartoma presentation.
It underscores the importance of considering spinal hamartoma in adult differential diagnoses, even in the absence of typical associated conditions.
Further research into the MRI characteristics of spinal hamartoma may improve diagnostic accuracy.
