    Summary

Large dental lesions do not necessarily compromise teeth. Endodontic surgery offers a viable solution when conventional root canal therapy is unsuccessful in healing the lesion.

    Area of Science:

    • Dentistry
    • Endodontics
    • Oral Surgery

    Background:

    • Large periapical lesions can pose a treatment challenge in endodontics.
    • Conventional root canal therapy (RCT) is the primary treatment for endodontic infections.
    • Treatment failure necessitates alternative or advanced therapeutic approaches.

    Observation:

    • Three clinical cases involving large periapical lesions were analyzed.
    • The teeth in question remained vital and functional despite the significant lesion size.
    • Initial treatment with conventional root canal therapy was attempted in all cases.

    Findings:

    • The presence of large periapical lesions did not inherently compromise the dental structures.
    • Conventional root canal therapy demonstrated limitations in achieving complete healing for these specific large lesions.
    • Endodontic surgery was successfully employed as a definitive treatment option.

    Implications:

    • Endodontic surgery should be considered a crucial treatment modality for persistent periapical lesions.
    • The long-term prognosis for teeth with large periapical lesions can be favorable with appropriate surgical intervention.
    • This approach highlights the potential to save teeth that might otherwise be considered non-restorable.
