Endoscopic Studies II: Thoracocentesis

2.1K
Thoracentesis(Thoracocentesis), commonly known as pleural tap, is a medical procedure where a 22 gauge needle is inserted into the pleural space, the area between the lung and chest wall. This procedure is commonly performed to diagnose or treat various respiratory disorders.
Description
Excess pleural fluid or air may accumulate in some respiratory disorders in the thoracic cavity. To treat pleural effusion, a physician conducts thoracentesis by carefully piercing the chest wall and entering...

Tracheostomy Suctioning II: Procedure

3.1K
Tracheostomy suctioning is a vital nursing procedure that involves removing secretions from the tracheostomy tube to maintain airway patency and prevent respiratory complications. Nurses need to understand the proper technique for tracheostomy suctioning to ensure patient safety and comfort. In this guide, we will outline the step-by-step process for performing tracheostomy suctioning, including preparing the sterile field, donning personal protective equipment (PPE), lubricating and connecting...

Chest Physiotherapy01:24

Chest Physiotherapy

2.6K
Chest Physiotherapy (CPT) is a therapeutic technique used in respiratory care to improve ventilation, clear bronchial secretions, and enhance the efficiency of respiratory muscles. This therapy includes three primary procedures: postural drainage, percussion, and vibration. It can be performed on spontaneously breathing patients and those who are intubated and mechanically ventilated.
Purpose
CPT is primarily used for patients with excessive bronchial secretions who have difficulty clearing...

Pneumothorax-II

1.5K
Pneumothorax is a medical condition defined by the buildup of air in the pleural space between the lungs and the chest wall. This accumulation of air can lead to partial or complete lung collapse, resulting in a range of clinical manifestations. Understanding the clinical presentation and effective management strategies is crucial for healthcare professionals in providing timely and appropriate care to individuals with pneumothorax.

Tracheostomy Suctioning I: Pre-Procedural Steps

3.3K
Tracheostomy suctioning is a critical procedure healthcare professionals perform to maintain a patent airway in patients with a tracheostomy tube. This procedure is necessary when secretions accumulate in the airway, causing respiratory distress. Here is a step-wise procedural guide for performing tracheostomy suctioning using an open system.
Equipment Required
First, gather all necessary equipment: a sterile suction catheter, a sterile disposable container, sterile gloves, a towel or...

Oxygen Delivering System II: Venturi Mask and Transtracheal Oxygen

3.3K
Oxygen therapy is a pivotal aspect of medical care, particularly for patients with respiratory ailments. Two prominent oxygen-delivering systems include the Venturi mask and the transtracheal oxygen catheter.
Venturi Mask
The Venturi mask, named after the Venturi effect, is designed to deliver precise oxygen concentrations. It consists of a large tube with an oxygen inlet that narrows down, causing a pressure drop that pulls air in through adjustable side ports. The mask is a lightweight,...

Summary

A novel high vacuum chest drainage system (HVCDS) showed comparable drainage to conventional systems but experienced clotting. High negative pressures caused minor hemorrhages, suggesting further study is needed for fragile structures.

Area of Science:

  • Cardiovascular Surgery
  • Thoracic Surgery
  • Medical Device Engineering

Background:

  • Chest drainage systems are crucial post-thoracic surgery.
  • High vacuum chest drainage systems (HVCDS) offer potential advantages but require safety evaluation.
  • Conventional chest drainage systems (CCDS) are the current standard.

Purpose of the Study:

  • To evaluate the safety and feasibility of a novel high vacuum chest drainage system (HVCDS).
  • To compare the hemodynamic influence of HVCDS versus conventional chest drainage systems (CCDS).

Main Methods:

  • Anesthetized pigs underwent sternotomy with drain placement.
  • Animals were assigned to either HVCDS or CCDS.
  • Simulated and standardized bleeding models were used with varying negative pressures.

Main Results:

  • Both HVCDS and CCDS demonstrated comparable drainage efficacy.
  • HVCDS exhibited significant clotting issues.
  • High negative pressures (-20 kPa and -40 kPa) induced focal epicardial and pulmonary hemorrhages.

Conclusions:

  • Neither HVCDS nor CCDS induced hemodynamic instability at tested pressures.
  • HVCDS demonstrated adequate drainage but suffered from clotting.
  • High negative pressures caused localized hemorrhages, warranting further investigation for long-term use and interaction with grafts.
