Background:

  • Primary selective IgM deficiency (sIgM) presents with low IgM, infections, and autoimmunity.
  • The underlying B-cell pathogenesis of sIgM remains poorly understood.

Purpose of the Study:

  • To investigate peripheral B-cell subsets and IgM expression in adult sIgM patients.
  • To assess B-cell differentiation and antibody-secreting capacity in vitro.

Main Methods:

  • Flow cytometry was used to analyze B-cell subsets and IgM expression in 31 sIgM patients.
  • ELISpot assays assessed antibody-secreting cells after in vitro differentiation in a subset of patients.

Main Results:

  • sIgM patients showed increased transitional B cells and decreased IgM-only, switched, and non-switched memory B cells.
  • Reduced membrane IgM expression on memory B cells and diminished B-cell differentiation/expansion capacity were observed.
  • Severely reduced IgM-secreting capacity was detected in some patients.

Conclusions:

  • Altered B-cell subsets and significant functional B-cell defects are characteristic of sIgM.
  • These findings may aid in diagnosis and patient subclassification for further pathogenetic studies.
