Summary

Patients with Crohn's disease (CD) show a significantly higher postoperative inflammatory response, measured by C-reactive protein (CRP), compared to colorectal cancer (CRC) patients. This enhanced response in CD patients may stem from a higher preoperative inflammatory state.

Purpose of the Study:

  • To compare the postoperative inflammatory response, using C-reactive protein (CRP) levels, in patients undergoing surgery for Crohn's disease (CD) versus colorectal cancer (CRC).

Main Methods:

  • A retrospective study comparing 112 CD patients (ileocaecal resection) with 112 CRC patients (laparoscopic right hemicolectomy).
  • Postoperative C-reactive protein (CRP) levels were monitored for the first 7 days as the primary outcome measure.

Main Results:

  • Crohn's disease patients exhibited an average 27% higher postoperative CRP level compared to colorectal cancer patients (p=0.02).
  • Significant elevations in CRP for CD patients were observed on postoperative Days 1, 4, 5, and 6.
  • Preoperative CRP levels partially contributed to the observed differences in postoperative CRP.

Conclusions:

  • Crohn's disease patients demonstrate a heightened postoperative inflammatory response after surgery.
  • This enhanced inflammatory response in CD may be initiated by a pre-existing higher inflammatory state.
