Summary

Autonomic dysfunction detected by the head-up tilt test (HUTT) predicts atrial fibrillation recurrence after radiofrequency catheter ablation (RFCA). A positive HUTT indicates a higher risk of AF recurrence post-ablation.

Background:

  • Autonomic dysfunction is linked to paroxysmal atrial fibrillation (PAF).
  • The head-up tilt test (HUTT) is a key diagnostic for autonomic dysfunction.
  • Radiofrequency catheter ablation (RFCA) is a treatment for PAF.

Purpose of the Study:

  • To investigate the association between HUTT results and atrial fibrillation (AF) recurrence after RFCA in PAF patients.
  • To identify predictors of AF recurrence following RFCA.

Main Methods:

  • Prospective enrollment of 488 consecutive PAF patients undergoing RFCA.
  • Performance of HUTT to assess autonomic function.
  • Follow-up to determine AF recurrence rates and analyze predictive factors using multivariate Cox regression.

Main Results:

  • A positive HUTT was observed in 31.6% of patients.
  • AF recurrence occurred in 33.4% of patients.
  • Positive HUTT was significantly associated with higher AF recurrence rates (41.7% vs. 26.5%, P < 0.001).
  • Positive HUTT, left atrial diameter, AF duration, and sleep apnea were independent predictors of recurrence.
  • RFCA success rates were lower in the HUTT-positive group (58.4%) compared to the HUTT-negative group (70.4%).

Conclusions:

  • A positive HUTT is a significant predictor of AF recurrence after RFCA for PAF.
  • HUTT can help stratify patients at higher risk for recurrence post-ablation.
