Background:

  • The Balance Error Scoring System (BESS) is a common clinical tool for assessing postural stability.
  • Traditional BESS scoring can be subjective and may suffer from floor or ceiling effects.
  • Objective, quantitative measures of postural stability are needed for improved clinical assessment.

Purpose of the Study:

  • To develop a biomechanically based quantification of the BESS using tablet-derived accelerometer and gyroscope data.
  • To validate the accuracy of tablet-based kinematic data against a 3D motion capture system.
  • To introduce a novel 3D metric of postural stability, the iBESS volume.

Main Methods:

  • Thirty-two healthy young adults performed the BESS with an iPad placed at the sacrum.
  • Kinematic data from the iPad were compared to a 3D motion capture system.
  • Calculated metrics included Peak-to-Peak (P2P), Normalized Path Length (NPL), Root Mean Squared (RMS), and a 3D iBESS volume using center of mass (CoM) movements.

Main Results:

  • iPad-derived kinematic data showed significant correlations with motion capture data (rho range, 0.37-0.94; P < 0.05).
  • The iBESS volume metric detected significant differences in postural stability across varying stance and surface conditions.
  • The iBESS volume was more sensitive to changes in postural stability than traditional BESS error scoring.

Conclusions:

  • Tablet-based kinematic data are sufficiently accurate for quantifying postural stability.
  • The iBESS volume offers a more sensitive measure of postural stability compared to traditional BESS scoring, especially in challenging conditions.
  • The iBESS metric is suitable for clinical and field-based assessments of postural stability.
