Summary

Aided auditory steady-state responses (ASSRs) can help predict hearing aid success. Comparing gains, not thresholds, is more accurate for hearing aid validation and cochlear implant decisions.

Purpose of the Study:

  • To compare the characteristics of aided and unaided auditory steady-state responses (ASSRs).
  • To evaluate the utility of aided ASSRs in predicting hearing aid success and informing cochlear implant decisions.

Main Methods:

  • 30 participants with moderate to profound hearing impairment underwent acoustic immittance, behavioral audiometry, and 40 Hz ASSR testing, both unaided and aided.
  • Biological calibration involved 16 normal-hearing and 17 severe-hearing-loss individuals.

Main Results:

  • Significant differences were found between unaided behavioral and ASSR thresholds across frequencies.
  • Aided behavioral and ASSR thresholds differed significantly at 1000 and 2000 Hz.
  • Aided ASSRs were recordable in some severe to profoundly hearing-impaired individuals, correlating with speech clarity and speech-reading ability.

Conclusions:

  • Comparing functional and ASSR gains is more accurate for hearing aid validation than comparing thresholds.
  • The inability to record aided ASSRs suggests a poor prognosis for hearing aid success.
  • Aided ASSR testing, with careful fitting and testing conditions, can be a valuable tool for hearing aid validation and cochlear implant decision-making.
