Dual antiplatelet therapy (DAPT) is linked to a significant drop in hemoglobin (Hb) levels. About half of patients on DAPT developed anemia or had their existing anemia worsen, especially if bleeding occurred.
Area of Science:
- Cardiology
- Hematology
- Clinical Pharmacology
Background:
- Dual antiplatelet therapy (DAPT), combining clopidogrel and acetylsalicylic acid, is widely used.
- The impact of DAPT on hemoglobin (Hb) levels in clinical practice requires further elucidation.
Purpose of the Study:
- To investigate the association between DAPT and changes in Hb levels.
- To determine the incidence of anemia or worsening anemia in patients undergoing DAPT.
Main Methods:
- Retrospective longitudinal analysis of 122 patients on DAPT for at least 6 months.
- Hb levels measured before and after DAPT initiation.
- Logistic regression analysis to identify factors associated with anemia.
Main Results:
- A significant decrease in mean Hb from 14.3 g/dl pre-DAPT to 12.8 g/dl post-DAPT (P<.001).
- Anemia prevalence increased from 9.1% to 45.9% post-DAPT.
- Post-treatment anemia was associated with bleeding and other causes of anemia.
Conclusions:
- DAPT is associated with a clinically significant reduction in Hb levels.
- Approximately half of patients on DAPT experienced new or worsened anemia.
- Concomitant bleeding and other anemia-inducing factors increase the risk of anemia in DAPT patients.
