Transcranial magnetic stimulation (TMS) shows promise for treating schizophrenia symptoms, including hallucinations and delusions. However, current evidence quality is low, preventing definitive recommendations for routine clinical use.
Area of Science:
- Neuroscience
- Psychiatry
- Clinical Trials
Background:
- Schizophrenia presents significant challenges with intractable symptoms despite available drug treatments.
- Transcranial magnetic stimulation (TMS) is an emerging alternative therapy for schizophrenia.
- Current consensus on TMS efficacy for schizophrenia in routine practice is lacking.
Purpose of the Study:
- To systematically review randomized controlled trials (RCTs) evaluating TMS for schizophrenia symptoms.
- To assess the effectiveness of TMS compared to sham or standard treatments.
Main Methods:
- Systematic literature review of RCTs comparing TMS with sham or standard treatment.
- Inclusion of 41 trials with 1473 participants.
- Data extraction and analysis of outcomes for global state and positive symptoms.
Main Results:
- Temporoparietal TMS showed significant improvements in global state and positive symptoms compared to sham TMS.
- Evidence quality was frequently suboptimal, with high or unascertainable risks of bias.
- Results were often graded as very low-quality evidence.
Conclusions:
- The current evidence is insufficient to definitively support or refute the routine use of TMS for schizophrenia.
- High-quality, definitive trials with rigorous reporting are necessary to establish TMS efficacy.
- Further research is needed to determine the role of TMS in clinical practice for schizophrenia management.
