Area of Science:

  • Dentistry
  • Periodontology
  • Oral Surgery

Background:

  • A 32-year-old patient presented with complete denudation of the buccal root of tooth #14, with Grade-I mobility.
  • The patient was unwilling to undergo extraction, presenting a clinical challenge requiring innovative treatment strategies.

Observation:

  • The case involved an unusual presentation and unexpected intraoperative conditions at the surgical site.
  • Established periodontal and endodontic techniques were modified to address the severe root exposure and bone deficiency.

Findings:

  • A combination of endodontic treatment, buccal root resection, osseous grafting, and a lateral pedicle flap was employed.
  • The tooth survived, exhibiting reduced mobility and complete soft-tissue coverage post-operatively.
  • Follow-up at 3 months and 1 year revealed uneventful healing and a stable gingival margin.

Implications:

  • This case demonstrates the successful application of modified surgical techniques for tooth preservation in complex periodontal defects.
  • The findings suggest that tailored, out-of-the-box approaches can achieve favorable outcomes in challenging dental salvage cases.
  • This approach offers a viable alternative to extraction, improving patient quality of life and maintaining dental function.
