Background:

A 32-year-old patient presented with complete denudation of the buccal root of tooth #14, with Grade-I mobility.
The patient was unwilling to undergo extraction, presenting a clinical challenge requiring innovative treatment strategies.

Observation:

The case involved an unusual presentation and unexpected intraoperative conditions at the surgical site.
Established periodontal and endodontic techniques were modified to address the severe root exposure and bone deficiency.

Findings:

A combination of endodontic treatment, buccal root resection, osseous grafting, and a lateral pedicle flap was employed.
The tooth survived, exhibiting reduced mobility and complete soft-tissue coverage post-operatively.
Follow-up at 3 months and 1 year revealed uneventful healing and a stable gingival margin.
