Clear cell odontogenic carcinoma (CCOC) is a rare jaw cancer. Surgical resection with fibula free flap reconstruction offers a good prognosis for large mandibular CCOC cases.

Background:

Clear cell odontogenic carcinoma (CCOC) is an exceptionally rare intraosseous malignancy of the jaw.
Only 81 cases have been documented in English literature, highlighting its rarity.

Observation:

A case of a 70-year-old woman with a large, painful, radiolucent mandibular lesion is presented.
The lesion extended from the canine to the angle area without detected lymph node or distant metastases.
The patient underwent wide surgical resection and reconstruction using a composite fibula free flap.

Findings:

Clear cell odontogenic carcinoma predominantly affects women aged 50-70 years, primarily in the mandible.
Common symptoms include painless swelling, pain, tooth loosening, and paresthesia.
