Summary

Primary testicular non-Hodgkin's lymphoma (NHL) is rare. This study of 8 patients found that chemoimmunotherapy with prophylactic intrathecal chemotherapy and contralateral testis irradiation effectively prevented relapse, with only one central nervous system recurrence.

Background:

  • Primary testicular lymphoma (PTL) is an aggressive extranodal non-Hodgkin's lymphoma (NHL).
  • Understanding PTL's characteristics and outcomes is crucial for effective management.

Purpose of the Study:

  • To analyze the clinicopathological features of PTL.
  • To evaluate treatment outcomes for PTL patients.

Main Methods:

  • Retrospective review of 339 NHL patient records from 2005-2013.
  • Identification and analysis of 8 PTL cases.
  • Assessment of treatment protocols including inguinal orchiectomy, chemoimmunotherapy (cyclophosphamide, doxorubicin, vincristine, prednisone plus rituximab), intrathecal prophylaxis, and contralateral testis irradiation.

Main Results:

  • Eight PTL patients were identified with an average age of 67.7 years.
  • All patients received chemoimmunotherapy; 4 received intrathecal prophylaxis and 6 received contralateral testis irradiation.
  • One patient (without intrathecal prophylaxis) relapsed in the central nervous system; no contralateral testis relapse occurred.

Conclusions:

  • Primary testicular NHL is an uncommon but significant clinical entity.
  • Combined chemoimmunotherapy with prophylactic intrathecal chemotherapy and contralateral testis irradiation appears effective in preventing PTL relapse.
  • Intrathecal prophylaxis is critical for preventing central nervous system involvement in PTL.
