Summary

Creatine kinase (CK) isoenzymes and isoforms in serum aid in diagnosing acute myocardial infarction and assessing coronary artery reperfusion. MB isoenzyme measurements offer high clinical sensitivity for critical cardiac patient management decisions.

Area of Science:

  • Biochemistry
  • Cardiology
  • Clinical Diagnostics

Background:

  • Serum creatine kinase (CK) exists as MM and MB isoenzymes, with multiple isoforms derived from tissue-specific forms.
  • Carboxypeptidase N converts released MM and MB isoforms from necrotic tissue into other serum forms.
  • CK isoforms are crucial biomarkers in cardiovascular disease and skeletal muscle disorders.

Purpose of the Study:

  • To review the role of CK isoforms in managing myocardial infarction (MI).
  • To discuss current and needed methods for CK isoform analysis.
  • To highlight the clinical utility of CK isoform measurements in cardiology.

Main Methods:

  • Review of existing literature on CK isoform analysis and clinical applications.
  • Discussion of analytical techniques including electrophoresis, isoelectric focusing, chromatofocusing, and liquid chromatography.
  • Evaluation of CK isoform measurement for diagnosing acute MI and monitoring reperfusion therapy.

Main Results:

  • MM isoforms are useful for early diagnosis of acute MI and assessing coronary reperfusion.
  • MB isoform measurements may offer higher clinical sensitivity, particularly in differentiating cardiac from skeletal muscle injury.
  • Current analytical methods require improvement in speed and sensitivity for emergency use.

Conclusions:

  • CK isoform analysis provides objective data for critical clinical decisions in cardiology.
  • Rapid and sensitive CK-MM and CK-MB isoform assays are needed for emergency settings.
  • Further development of analytical techniques is essential for effective clinical implementation.
