Purpose of the Study:

  • To quantify cannabinoid receptor-1 (CB1R) availability in the brains of male schizophrenia (SCZ) patients.
  • To compare CB1R availability between SCZ patients and healthy controls (HCs).
  • To explore the influence of antipsychotic treatment and tobacco use on CB1R availability in SCZ.

Main Methods:

  • Positron emission tomography (PET) scans were performed on 25 male SCZ subjects and 18 age-matched male HCs.
  • A selective CB1R radiotracer, [(11)C]OMAR, was used to measure regional volume of distribution (VT) as a proxy for CB1R availability.
  • Kinetic modeling of PET data was employed, with analyses covaried for age and body mass index.

Main Results:

  • SCZ subjects exhibited significantly lower composite [(11)C]OMAR VT compared to HCs (~12% reduction).
  • Reduced CB1R availability was observed in specific brain regions including the amygdala, caudate, and hippocampus in SCZ patients.
  • A gradient of CB1R availability was noted: HCs > antipsychotic-treated SCZs > antipsychotic-free SCZs, and HCs > SCZ smokers/nonsmokers.

Conclusions:

  • Male SCZ subjects demonstrate lower CB1R availability than HCs.
  • Antipsychotic medications and tobacco use may be associated with increased CB1R availability in SCZ.
  • These findings support the role of eCB system alterations in the pathophysiology of schizophrenia.
