Background:

  • Testicular epidermoid cysts are rare neoplasms, comprising approximately 1% of all testicular tumors.
  • They represent a distinct clinicopathological entity within germ cell tumors of the testis.

Observation:

  • Two cases of testicular epidermoid cysts in young males (18 and 19 years old) presented as painless testicular lesions.
  • Diagnostic workup included testicular ultrasound, revealing hypoechoic nodules in both instances.

Findings:

  • Despite initial suspicion of testicular neoplasm, pathological examination confirmed epidermoid cysts post-inguinal orchiectomy.
  • Surgical intervention included orchiectomy with concurrent testicular prosthesis placement.

Implications:

  • Testicular epidermoid cysts are uncommon but benign, necessitating accurate diagnosis to avoid overtreatment.
  • When suspected, imaging (ultrasound, MRI) and tumor markers are crucial; parenchyma-sparing surgery should be considered.
