Sleep Apnea01:21

Sleep Apnea

766
Sleep apnea is a condition where breathing stops intermittently during sleep, often leading to significant health issues. Each episode can last from 10 to 20 seconds or more and is frequently accompanied by a brief arousal from sleep. This disturbance, largely unnoticed by the individual, can lead to severe daytime fatigue. Commonly, individuals seek help after being informed by their partners about loud snoring and noticeable breathing pauses during sleep.
The condition is more prevalent among...

Cardiopulmonary Resuscitation II: ACLS Airway Management

998
Airway management is a key skill in emergency and critical care settings, as maintaining a clear airway is essential for adequate oxygenation and ventilation.Head Tilt-Chin Lift TechniqueThe head tilt-chin lift maneuver is an essential technique primarily used in patients without suspected cervical spine injuries. To perform this maneuver, one hand is placed on the patient’s forehead, and gentle pressure is applied backward to tilt the head. The fingertips of the other hand are positioned...

Suctioning the Nasopharyngeal Airway01:29

Suctioning the Nasopharyngeal Airway

5.3K
Nasopharyngeal suctioning is a procedure to remove secretions from the upper part of the respiratory tract that the patient cannot clear independently. It helps maintain airway patency and prevents complications such as aspiration pneumonia.

Epistaxis01:30

Epistaxis

727
Epistaxis, or nosebleeds, occurs when small, swollen blood vessels in the nasal mucous membrane rupture. Typically, the anterior septum is the primary site of occurrence.
Etiology
Possible causes of this condition include high blood pressure, trauma, low humidity, upper respiratory tract infections, allergies, foreign bodies, nasal inhalation of corticosteroids or illicit drugs, excessive use of decongestant nasal sprays, facial or nasal surgery, anatomic malformation, tumors, or systemic...

Hyperpnea and Hyperventilation01:25

Hyperpnea and Hyperventilation

3.9K
Hyperventilation refers to a higher-than-normal rate and depth of breathing, often associated with anxiety attacks. This excessive breathing surpasses the body's need to expel CO2, leading to a condition known as hypocapnia - an unusually low level of carbon dioxide in the blood. Hypocapnia can constrict cerebral blood vessels, reducing blood flow to the brain, which may result in dizziness or fainting. Early signs include tingling and muscle spasms in the hands and face, caused by falling...

Suctioning the Oropharyngeal Airway

1.5K
In preparing for oropharyngeal airway suctioning, a nurse must gather all necessary equipment, including a suction unit with tubing, a prepackaged suction kit, sterile gloves, water or saline for irrigation, a water-soluble lubricant, and additional personal protective equipment (such as a gown, mask, and goggles) to control infections.
After assembling the equipment, the nurse should practice hand hygiene and don appropriate PPE according to infection control guidelines to avoid the...

Summary

Vibratory angioedema (VA) from snoring is likely under-diagnosed. Continuous positive airway pressure (CPAP) therapy effectively controlled symptoms in a patient with sleep apnea, suggesting a new treatment approach.

Area of Science:

  • Dermatology
  • Sleep Medicine
  • Allergy and Immunology

Background:

  • Vibratory angioedema (VA) is a rare physical urticaria triggered by vibration, causing itching and swelling.
  • Current management focuses on vibration avoidance, but diagnosis can be challenging.
  • This report highlights a potential undiagnosed cause of VA and a novel treatment.

Observation:

  • A 70-year-old woman presented with recurrent nocturnal tongue and throat swelling over three years.
  • She also reported hand itching and swelling when driving and severe snoring.
  • A sleep study revealed severe sleep apnea with significant snoring.

Findings:

  • A diagnosis of vibratory angioedema induced by oropharyngeal vibration from snoring was made.
  • Continuous positive airway pressure (CPAP) therapy led to complete symptom resolution.

  • Standard angioedema workup, including complement levels, was unremarkable.
  • Implications:

    • Snoring-induced VA may be an under-diagnosed condition.
    • A thorough patient history is crucial for diagnosis.
    • CPAP therapy offers a potential novel treatment for snoring-induced VA by eliminating oropharyngeal vibration.
