Epstein-Barr virus (EBV) can cause joint pain and inflammation, particularly in women of childbearing potential. This condition may mimic rheumatoid arthritis and sometimes requires long-term treatment.

Background:
- Epstein-Barr virus (EBV) is a common human herpesvirus.
- Joint involvement is an uncommon manifestation of EBV infection.
Observation:
- Six cases of EBV infection with joint involvement were observed between February 2011 and January 2012.
- Patients were women aged 25-75, with four of childbearing potential.
- Clinical presentations included rheumatoid arthritis-like patterns, polyarthralgia, and asymmetrical oligoarthritis.
Findings:
- Diagnosis was made by exclusion and negative IgM for other herpesviruses.
- EBV-induced joint involvement was more prevalent in women of childbearing potential.
- Presentations were heterogeneous, predominantly inflammatory joint pain.
Implications:
- Symmetrical polyarthritis due to EBV can become chronic.
- Disease-modifying anti-rheumatic drugs may be required for chronic EBV-related arthritis.
- Highlights the diverse clinical spectrum of EBV infection.
