Infection-related hospitalizations are common in home hemodialysis (HHD) patients, often stemming from non-access issues. Different HHD methods did not increase hospitalization risk for these infections.
Area of Science:
- Nephrology
- Infectious Diseases
- Public Health
Background:
- Infection is a leading cause of hospitalization and mortality in hemodialysis (HD) patients.
- Limited research exists on infection-related hospitalizations specifically within the home HD (HHD) population.
Purpose of the Study:
- To investigate the spectrum of infections leading to hospitalization in HHD patients.
- To evaluate the impact of different HHD modalities, including daily (DHD) and conventional (CHD) home HD, on infection-related hospitalizations.
Main Methods:
- Analysis of a large cohort of HHD patients.
- Abstraction of infection-related hospitalization data from July 2005 to August 2010.
- Statistical analysis of demographics, dialysis vintage, and dialysis modality.
Main Results:
- Infection-related hospitalizations constituted the primary cause (35.8%) of all hospitalizations in HHD patients.
- Non-access-related infections were more frequent than access-related infections (1.7:1).
- Rates for soft-tissue infection, pneumonia, and sepsis ranged from 0.85 to 1.82 per 100 person-years; access-related infections were the main cause of access-related hospitalizations (34.8%).
Conclusions:
- Infection-related hospitalizations are a frequent occurrence in the HHD population.
- A wide array of infections, many not linked to dialysis access, contribute to hospitalizations in HHD patients.
- The specific HHD modality (DHD or CHD) was not found to be associated with an increased risk of infection-related hospitalizations.
