Summary

Choroidal thickness in Best vitelliform macular dystrophy (VMD) changes with disease stage. Choroid is thicker in early VMD stages and thins significantly in later atrophic stages, correlating with vision loss.

Background:

  • Best vitelliform macular dystrophy (VMD) is a genetic condition affecting central vision.
  • Understanding choroidal changes is crucial for VMD progression assessment.
  • Current knowledge on choroidal thickness variations in VMD stages is limited.

Purpose of the Study:

  • To investigate and quantify changes in choroidal thickness across different stages of Best vitelliform macular dystrophy (VMD).
  • To determine the correlation between subfoveal choroidal thickness (SFCT) and best-corrected visual acuity (BCVA) in VMD patients.

Main Methods:

  • Prospective enrollment of 34 VMD patients and 20 healthy controls.
  • Ophthalmologic examination including BCVA, biomicroscopy, and spectral domain optical coherence tomography (SD-OCT).
  • Gass classification used to stage VMD; SFCT measured and analyzed against VMD stage and BCVA.

Main Results:

  • Subfoveal choroidal thickness (SFCT) was similar to controls in Stage 1 VMD.
  • Stages 2 and 3 showed significantly increased SFCT compared to controls.
  • Stages 4 and 5 exhibited progressively reduced SFCT, with Stage 5 significantly thinner than controls and correlated with decreased BCVA.

Conclusions:

  • Choroidal thickness significantly varies with VMD stage, increasing in early stages and decreasing in later atrophic/cicatricial stages.
  • Thinning of the choroid in advanced VMD stages is associated with reduced visual acuity.
  • Further long-term studies are needed to fully elucidate morphofunctional alterations in VMD.
