Background:

  • The necessity of cerebral angiography before carotid endarterectomy is debated.
  • Carotid duplex scanning is a common non-invasive imaging technique.

Observation:

  • A retrospective review of 100 patients compared cerebral angiography with carotid duplex scanning.
  • Eighty patients underwent carotid endarterectomy, while 20 did not.
  • Arteriography revealed findings not detected by duplex scanning in some cases.

Findings:

  • Cerebral angiography altered management in 8% of the 100 patients reviewed.
  • Three patients avoided unnecessary surgery due to arteriography revealing mistaken diagnoses.
