Fractures: Bone Repair

6.6K
Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the...

    Summary

This study demonstrates that using a mini-plate via a medial approach effectively treats first metatarsal shaft comminuted fractures. Patients achieved good bone union, pain relief, and functional recovery, with excellent cosmetic outcomes.

    Area of Science:

    • Orthopedic Surgery
    • Traumatology
    • Podiatric Medicine

    Background:

    • First metatarsal shaft comminuted fractures are complex injuries requiring effective treatment.
    • Achieving anatomical reduction and stable fixation is crucial for functional recovery and foot arch maintenance.

    Purpose of the Study:

    • To evaluate the effectiveness and surgical technique of using a mini-plate via a medial approach for treating first metatarsal diaphysis comminuted fractures.
    • To assess operative methods and outcomes for this specific fracture type.

    Main Methods:

    • A cohort of 15 patients with first metatarsal shaft comminuted fractures (AO classification 81(T)-C2) were treated between January 2012 and January 2013.
    • Surgical intervention involved open reduction and internal fixation with a mini-plate through a medial approach.
    • Patients were followed up for an average of 18 months post-surgery.

    Main Results:

    • All incisions healed primarily without infection.
    • All fractures achieved bone union within an average of 11.5 weeks.
    • Patients experienced no pain, could bear full weight, and showed good functional recovery with an average AOFAS score of 85.2.

    Conclusions:

    • The mini-plate technique via a medial approach provides effective treatment for first metatarsal comminuted fractures.
    • This method ensures stable internal fixation and anatomical reduction, crucial for restoring foot form and function.
    • The medial approach offers improved cosmetic results and adequate surgical exposure.
