Summary

Transvenous embolization is a safe and effective endovascular treatment for pediatric cerebral arteriovenous malformations (AVMs). This approach achieved complete AVM obliteration in all treated children, with no major complications reported.

Background:

  • Transvenous embolization (TVA) is an emerging endovascular technique for treating cerebral arteriovenous malformations (AVMs).
  • Limited data exists on the feasibility and outcomes of TVA in pediatric populations.

Purpose of the Study:

  • To evaluate the safety and efficacy of the transvenous approach (TVA) for treating cerebral AVMs in pediatric patients.
  • To report the initial experience with TVA in children younger than 18 years.

Main Methods:

  • A cohort of 7 pediatric patients with cerebral AVMs underwent TVA between January 2012 and January 2014.
  • TVA was used alone or combined with arterial embolization.
  • Outcomes were assessed based on AVM characteristics, clinical results, and angiographic obliteration at 6-month follow-up.

Main Results:

  • Complete anatomic exclusion of AVMs was achieved in all 7 patients.
  • Hemorrhage was the primary presentation in 100% of cases.
  • No hemorrhagic or thromboembolic events occurred; no recurrence was observed during a mean follow-up of 20.2 months.

Conclusions:

  • Transvenous embolization is a feasible and safe endovascular treatment for pediatric cerebral AVMs.
  • This technique offers a viable alternative for specific pediatric AVMs unsuitable for microsurgery or radiosurgery.
