Summary

Systemic sclerosis (SSc) patients may develop squamous cell lung cancer (SqLC), often with interstitial lung disease (ILD), years after their SSc diagnosis. Early recognition of SqLC in SSc patients, even non-smokers, is crucial for timely treatment.

Background:

  • Systemic sclerosis (SSc) is an autoimmune disease with potential multi-organ involvement.
  • Lung cancer, particularly squamous cell lung cancer (SqLC), is a known complication in SSc patients.
  • Interstitial lung disease (ILD) is frequently observed in SSc patients.

Observation:

  • A case of SqLC with underlying ILD in an SSc patient is presented.
  • A literature review identified 21 cases of SqLC associated with SSc.
  • SqLC in SSc patients occurs at a younger average age (57 years) compared to the general population.

Findings:

  • SqLC associated with SSc can occur in never or light smokers.
  • Two-thirds of SSc-associated SqLC cases involve ILD, often developing within the diseased lung area.
  • SqLC typically manifests approximately 12 years after the initial SSc diagnosis.

Implications:

  • Understanding these characteristics aids in the early diagnosis and management of SqLC in SSc patients.
  • Physicians should maintain a high index of suspicion for SqLC in SSc patients, especially those with ILD.
  • This knowledge can improve follow-up protocols and treatment strategies for SSc patients at risk for lung cancer.
