Background:

  • Deep neck infections (DNIs) present acutely and can lead to severe complications.
  • Pediatric DNIs are infrequently reported in Taiwan, necessitating further clinical and etiological understanding.

Purpose of the Study:

  • To investigate the clinical manifestations, diagnostic clues, and etiology of pediatric deep neck infections in Taiwan.
  • To analyze the common pathogens and treatment strategies for DNIs in pediatric patients.

Main Methods:

  • Retrospective chart review of pediatric patients (≤18 years) diagnosed with DNI between January 2005 and December 2014.
  • Classification of DNIs based on anatomical spaces: retropharyngeal, parapharyngeal, peritonsillar, submandibular, and multispace abscesses.

Main Results:

  • 52 pediatric DNIs were identified; parapharyngeal space abscesses were most common (42.3%).
  • Upper respiratory tract infections preceded 30.8% of cases. Neck mass/swelling (82.7%) and fever (75%) were primary symptoms.
  • Staphylococcus aureus was the most common pathogen; amoxicillin-clavulanic acid was the frequent antibiotic choice. No long-term morbidity or mortality observed.

Conclusions:

  • DNI should be considered in pediatric patients presenting with neck mass or swelling.
  • Low culture rates in Taiwan may impact pathogen identification; Gram staining and acid-fast staining are crucial early diagnostic steps.
  • Recurrent DNIs may indicate an underlying bronchogenic cyst, requiring surgical excision for cure.
