Nasal continuous positive airway pressure (CPAP) therapy for obstructive sleep apnea can cause severe nosebleeds. This case highlights the potential for epistaxis as a serious side effect of CPAP treatment.

Area of Science:

  • Cardiorespiratory Medicine
  • Sleep Medicine

Background:

  • Obstructive sleep apnea (OSA) is a common sleep disorder.
  • Continuous positive airway pressure (CPAP) is a primary treatment for OSA.
  • Right heart failure can be secondary to OSA.

Observation:

  • A 75-year-old male patient with OSA and secondary right heart failure was initiated on nasal CPAP.
  • The patient subsequently experienced a life-threatening epistaxis (nosebleed).

Findings:

  • The severe epistaxis necessitated nasal packing and hospitalization.
  • The event was attributed to the drying effects of nasal CPAP therapy.

Implications:

  • Nasal CPAP may cause significant epistaxis in OSA patients.

Related Experiment Videos

  • Clinicians should consider epistaxis as a potential adverse effect of CPAP.
  • Further research into managing CPAP-induced epistaxis is warranted.
