Summary

Ectopic and supernumerary parathyroid glands are common in renal hyperparathyroidism surgery. Preoperative imaging and extensive exploration can improve surgical outcomes for secondary hyperparathyroidism.

Background:

Secondary hyperparathyroidism (SHPT) is a common complication in patients with chronic renal failure.
Medical therapy is often insufficient for advanced SHPT, necessitating surgical intervention.
Identifying and removing all abnormal parathyroid glands is crucial for successful treatment.

Purpose of the Study:

To determine the frequency of ectopic and supernumerary parathyroid glands in patients undergoing surgery for renal hyperparathyroidism.
To evaluate the impact of extensive surgical exploration and preoperative imaging on surgical outcomes.

Review of preoperative imaging (ultrasound, Tc scintigraphy) and intraoperative findings.

Main Results:

Ectopic and supernumerary parathyroid glands were identified in a significant proportion of patients requiring reoperation (7%).
Supernumerary glands were found in 11% of patients during the first operation and 3% during the second.
Persistent high PTH levels after initial surgery indicated the presence of missed or ectopic glands.

Conclusions:

Extensive cervical exploration is essential for identifying all abnormal parathyroid glands during SHPT surgery.
Preoperative imaging, including parathyroid ultrasound and scintigraphy, can aid in locating ectopic and supernumerary glands.
Improved surgical strategies incorporating thorough exploration and imaging can reduce the need for repeat surgeries for secondary hyperparathyroidism.
