Summary

Cat-scratch disease (CSD) can present with varied posterior segment eye issues beyond the typical macular star. Early screening for Bartonella henselae is crucial for patients with optic nerve or retinal inflammation.

Area of Science:

  • Ophthalmology
  • Infectious Diseases
  • Retinal Imaging

Background:

  • Cat-scratch disease (CSD) is a systemic infection.
  • Neuroretinitis with a macular star is a common posterior segment manifestation.
  • This study highlights diverse posterior segment presentations of CSD.

Purpose of the Study:

  • To describe the heterogeneity of posterior segment manifestations in Cat-scratch disease.
  • To emphasize varied clinical presentations beyond the classic macular star.
  • To inform clinicians about broader diagnostic considerations for CSD.

Main Methods:

  • Retrospective case series.
  • Analysis of consecutive patients with posterior segment CSD over 5 years (2010-2015).
  • Data collected from two ophthalmological centers.

Main Results:

  • Twelve patients (17 eyes) with posterior segment CSD were identified.
  • All patients (100%) had optic nerve edema; 67% had focal chorioretinitis.
  • Only 25% initially showed a macular star, rising to 50% later; 42% had bilateral disease.

Conclusions:

  • The classic neuroretinitis with macular star is not the sole presentation of CSD.
  • Clinicians should consider screening for Bartonella henselae in patients with papillitis or focal chorioretinitis.
  • Recognizing diverse ocular manifestations is key for timely CSD diagnosis and treatment.
