Lay health worker outreach increased discussions about cholesterol treatment for cardiovascular disease prevention. However, this did not significantly increase statin prescribing, highlighting the need for clinician-focused interventions.

Background:

Purpose of the Study:

  • To evaluate the effectiveness of a lay health worker outreach program on statin prescribing for primary CVD prevention.
  • To assess the impact of individualized risk communication on patient-clinician discussions about cholesterol treatment.

Main Methods:

  • A pragmatic randomized controlled trial was conducted at community health centers.
  • Participants included men (≥45 years) and women (≥45 years) with a 10-year coronary heart disease risk of at least 10%, excluding those with existing CVD or diabetes.
  • The intervention group received telephone and mailed outreach from lay health workers, while the control group received standard care.

Main Results:

  • A significantly higher proportion of the intervention group (26.8%) discussed cholesterol treatment with a clinician compared to the control group (11.6%).
  • Statin prescribing increased in the intervention group (10.1%) versus the control group (6.0%), but this difference was not statistically significant.
  • There was no significant difference in low-density lipoprotein (LDL)-cholesterol levels between the groups, and most patients did not repeat lipid levels.

Conclusions:

  • Lay outreach effectively increased patient-clinician discussions regarding cholesterol treatment for CVD prevention.
  • The intervention did not lead to a significant increase in statin prescribing rates.
  • Future interventions should combine patient outreach with strategies to encourage clinicians to adhere to evidence-based cholesterol treatment guidelines.
