Background:

  • Flow diversion is an emerging treatment for intracranial aneurysms.
  • Long-term outcomes and the nature of residual aneurysmal tissue remain areas of uncertainty.
  • Standard follow-up imaging may not capture all aspects of treatment response.

Observation:

  • A 22-year-old male with a dissecting anterior cerebral artery aneurysm presented with subarachnoid hemorrhage.
  • Treatment involved a Pipeline Embolization Device (PED).
  • The patient self-discontinued dual antiplatelet therapy, leading to follow-up imaging.

Findings:

  • MRI revealed reperfusion in an aneurysmal remnant despite initial non-perfusion.
  • Angiography demonstrated new channels reconstituting flow distal to the PED.
  • Central PED thrombosis was not visualized on MRI, indicating potential limitations of MRI in assessing complete occlusion.

Implications:

  • Non-regressed, non-opacified aneurysmal remnants may be biologically active and prone to recanalization.
  • Current MRI protocols may underestimate residual disease after flow diversion.
  • This highlights the need for multimodal imaging or alternative follow-up strategies for intracranial aneurysms treated with flow diversion.
