Area of Science:

  • Gastroenterology
  • Pharmacology

Background:

  • Chronic diarrhea often stems from non-infectious causes.
  • Definitive treatments are not always available, necessitating symptomatic drug therapy.

Purpose of the Study:

  • To review and summarize pharmacologic agents used for symptomatic treatment of chronic diarrhea.
  • To highlight the mechanisms and applications of various drug classes in managing chronic diarrhea.

Main Methods:

  • Literature review of pharmacologic agents for chronic diarrhea.
  • Categorization of drugs based on their mechanisms of action and therapeutic uses.

Main Results:

  • Loperamide reduces peristalsis and secretion.
  • Cholestyramine is a first-line treatment for bile acid diarrhea.
  • 5-HT3 receptor antagonists benefit irritable bowel syndrome (IBS) with diarrhea.
  • Diosmectite acts as an absorbent for various types of diarrhea.
  • Antispasmodics alleviate diarrheal symptoms and abdominal pain.
  • Rifaximin is effective for IBS-associated diarrhea and small intestinal bacterial overgrowth.
  • Budesonide is effective for lymphocytic and collagenous colitis.

Conclusions:

  • A range of pharmacologic agents are available for symptomatic management of chronic diarrhea.
  • Proper selection of agents based on their mechanisms of action is essential for effective treatment.
  • Further research may be needed to clarify the role of certain agents, such as probiotics, in chronic diarrhea treatment.
