Background:

  • Transplant nephrectomy (TN) is a critical procedure following kidney transplantation, carrying substantial risks.
  • Understanding the indications and outcomes of TN is vital for optimizing subsequent renal transplantation success.

Purpose of the Study:

  • To identify the primary reasons for transplant nephrectomy (TN) at a UK transplant center.
  • To determine the complication rates, impact on re-listing, and re-transplantation success after TN.

Main Methods:

  • Retrospective analysis of all transplant nephrectomies (TN) performed between January 2000 and December 2011.
  • Detailed review of patient demographics, allograft failure causes, and TN indications.

Main Results:

  • Out of 602 kidney transplants, 42 TNs were performed on 38 patients (6%).
  • Graft thrombosis was the most common indication for TN (50%), particularly within the first month post-transplant.
  • In-hospital mortality was 9.5%, morbidity was 31%, and 7 out of 19 listed patients were successfully re-transplanted.

Conclusions:

  • Transplant nephrectomy (TN) is associated with significant morbidity and mortality but does not preclude future kidney re-transplantation.
  • The findings underscore the need for a national registry to better track transplant failures and TN outcomes.
