Background:

  • Traditional endovascular and open surgical bypass options are often precluded by severe anatomical limitations and comorbidities.

Observation:

  • A case of CMI is presented where standard surgical and endovascular interventions were not possible.
  • Severe aortoiliac occlusive disease prohibited endovascular access and retrograde bypass.
  • A thoracoabdominal aortic aneurysm precluded standard antegrade bypass.

Findings:

  • An extra-anatomic axillo-mesenteric reconstruction was successfully performed.
  • This approach provided a robust and viable alternative for mesenteric revascularization.

Implications:

  • Axillo-mesenteric bypass represents a crucial alternative surgical technique for CMI.
  • This method expands treatment options for patients with challenging vascular anatomies.
  • It offers a potential solution for complex cases unsuitable for conventional therapies.
