[Allergic fungal rhinosinusitis]

Background:

• Allergic fungal sinusitis (AFS) diagnosis and management remain challenging.
• This study presents a case of AFS and reviews current literature.

Observation:

• A 45-year-old male presented with steroid-resistant sinonasal polyposis.
• Imaging revealed sinus opacification with maxillary sinus lysis.
• Laboratory tests showed hypereosinophilia and elevated IgE levels.

Findings:

• Aspergillus fumigatus was identified through mycological investigation.
• Histopathology confirmed allergic mucin and eosinophilic inflammation.
• Endoscopic ethmoidectomy and oral steroids led to a favorable outcome.

Implications:

• Accurate diagnosis of AFS necessitates integrating clinical, radiological, histopathological, immunoallergological, and mycological data.
• Treatment involves endoscopic surgery combined with corticosteroid therapy.
• This approach can prevent disease progression and recurrence.
