Summary
This is the first reported case of cervical adenocarcinoma exhibiting a stromal micropapillary pattern (SMP). This rare uterine cervical cancer subtype showed MUC-1 expression and lymph node metastasis, but the patient remained disease-free post-treatment.

Background:
- Adenocarcinoma with a stromal micropapillary pattern (SMP) is a recognized subtype in various organs.
- Its occurrence in the uterine cervix has not been previously documented.
Observation:
- A case of cervical adenocarcinoma with SMP was identified in a 54-year-old Japanese woman presenting with abnormal vaginal bleeding.
- Cervical cytology revealed adenocarcinoma with features suggestive of serous adenocarcinoma, characterized by small tumor cell clusters and high nucleus:cytoplasm ratios.
- Microscopic examination confirmed predominantly adenocarcinoma with SMP, exhibiting membranous mucin-1 (MUC-1) expression on cell clusters and extensive lymph node metastasis.
Findings:
- The tumor was diagnosed as cervical adenocarcinoma with SMP and coexistent squamous cell carcinoma in situ.
- Pathological staging was T1b1N1M1, stage IVB.
- The patient received adjuvant chemotherapy and remained disease-free with no local recurrence or distant metastasis at 48 months post-surgery.
Implications:
- This case represents the first documented instance of cervical adenocarcinoma with SMP.
- The findings expand the known organ distribution of SMP.
