Symptom Dimensions, Smoking and Impulsiveness in Obsessive-Compulsive Disorder

Smoking rates vary significantly across obsessive-compulsive disorder (OCD) symptom dimensions. The symmetry-counting-repeating-ordering (S+C+R+O) dimension shows the highest smoking prevalence, not explained by impulsiveness.
Area of Science:
- Psychiatry
- Neuroscience
- Behavioral Science
Background:
- Obsessive-compulsive disorder (OCD) presents with distinct symptom dimensions, potentially linked to unique neurobiological underpinnings.
- Smoking behavior, while studied in other psychiatric conditions, remains underexplored in relation to specific OCD symptom clusters.
Purpose of the Study:
- To investigate the association between specific OCD symptom dimensions and smoking behavior.
- To explore potential neurobiological or psychological factors, such as impulsiveness, that might mediate this relationship.
Main Methods:
- A cohort of 167 OCD patients were assessed for symptom dimensions (washing, taboo thoughts, symmetry-counting-repeating-ordering [S+C+R+O]).
- Smoking status, nicotine dependence (Fagerström Test for Nicotine Dependence), depression (Hamilton Depression Rating Scale-17 Items), anxiety (Beck Anxiety Inventory), and impulsiveness (Barratt Impulsiveness Scale-11) were evaluated.
Main Results:
- Significant differences in smoking prevalence were observed across OCD symptom dimensions (p=0.009).
- The S+C+R+O group exhibited the highest smoking ratio (68.2%), contrasting with the washing group (30%) and taboo thoughts group (37.14%).
- Post hoc analyses confirmed higher smoking rates in the S+C+R+O group compared to washing (p=0.004) and taboo thoughts (p=0.007) groups.
Conclusions:
- OCD exhibits heterogeneity regarding smoking behavior, with symptom dimensions playing a significant role.
- Impulsiveness levels did not significantly differ across symptom dimensions, failing to explain the observed smoking variations.
- Nicotine addiction severity among smokers with OCD was consistent across different symptom dimensions.
