Summary

This case report highlights a rare instance of ungueal leishmaniasis presenting as chronic paronychia. Early recognition of atypical leishmaniasis presentations is crucial for effective treatment.

Background:

Chronic paronychia is a persistent inflammatory condition of the nail folds.
Cutaneous leishmaniasis, caused by Leishmania parasites, typically presents with skin lesions.
Atypical presentations of leishmaniasis can mimic other dermatological disorders, posing diagnostic challenges.

Observation:

A 34-year-old man presented with a 6-month history of inflamed proximal nail fold of the left thumb.
The condition was resistant to standard antibiotic and antifungal treatments.
Surgical excision of the nail fold was performed for diagnosis.

Findings:

Histopathological examination revealed epithelioid granulomas with giant cells.
The presence of Leishmania amastigotes confirmed the diagnosis of ungueal leishmaniasis.

Timely diagnosis and appropriate treatment are essential to manage this rare manifestation of leishmaniasis.
