This pilot study shows that a Hospital at Home (HaH) model using virtual physician visits is safe, feasible, and satisfactory. The HaH model may significantly reduce hospital readmissions for patients.
Area of Science:
- Health Services Research
- Telemedicine
- Geriatric Care
Background:
- Traditional acute care hospitalization presents challenges in patient satisfaction and readmission rates.
- Innovative care models are needed to improve healthcare delivery and patient outcomes.
Purpose of the Study:
- To assess the safety, feasibility, and efficacy of a substitutive Hospital at Home (HaH) model.
- To evaluate a model utilizing 2-way biometrically enhanced tele-video for physician care over a 34-day episode.
Main Methods:
- A prospective, nonrandomized, quasi-experimental study design.
- Comparison of 50 patients in HaH with 52 patients in traditional acute care hospital settings.
- Data collected from medical records and patient surveys.
Main Results:
- HaH patients reported higher satisfaction across multiple domains.
- Similar rates of illness-specific quality standards were met in both groups.
- A trend towards improved activities of daily living was observed in HaH patients.
- HaH patients showed a significant reduction in hospital readmissions at 90 days (aOR 0.39).
Conclusions:
- The described Hospital at Home model is safe, feasible, and highly satisfactory for patients.
- This model, incorporating virtual physician visits and extended care episodes, may substantially decrease hospital readmissions.
- Findings suggest a scalable approach to home-based acute care delivery.
