Background:

  • Refractory epilepsy affects nearly 30% of patients despite multiple antiepileptic drug (AED) regimens.
  • Medically refractory epilepsy is defined as failure to respond to at least two AEDs.
  • Early surgical intervention for epilepsy may improve intellectual and social outcomes in children.

Purpose of the Study:

  • To evaluate the benefits and adverse effects of subpial transection for partial-onset and generalized tonic-clonic seizures.
  • To assess the efficacy of subpial transection in pediatric and adult populations.

Main Methods:

  • Comprehensive literature search of Cochrane Epilepsy Group Register, CENTRAL, and MEDLINE up to June 2015.
  • Inclusion of randomized and quasi-randomized parallel-group studies, regardless of blinding.
  • Independent screening and data extraction by two review authors.

Main Results:

  • No relevant studies were identified that met the inclusion criteria.
  • The review could not find any trials investigating subpial transection for epilepsy.

Conclusions:

  • There is currently no evidence to support or refute the use of subpial transection for medically refractory epilepsy.
  • High-quality randomized controlled trials are essential to establish the efficacy and safety of this surgical procedure.
  • Further research is required to guide clinical decision-making for patients with uncontrolled epilepsy.
