    Area of Science:

    • Oncology
    • Head and Neck Surgery
    • Radiology

    Background:

    • Review of 61 literature cases of aggressive ossifying fibroma (OF) in the paranasal sinus.
    • Description of an additional case in an 11-year-old girl.

    Observation:

    • OF in the paranasal sinus presents with similar radiographic features.
    • These tumors exhibit aggressive clinical behavior.
    • High recurrence rates are noted after incomplete treatment.

    Findings:

    • The term 'active OF' accurately describes this entity, simplifying histologic confusion.
    • Aggressive OF requires prompt surgical intervention.

    Implications:

    • Head and neck surgeons must be aware of this diagnosis.
    • En bloc resection is the recommended surgical approach.
    • Timely diagnosis and treatment are vital for managing aggressive OF.
