Summary

Adult epiglottitis, or supraglottitis, often presents mildly. Most adult cases improve with medical treatment, rarely requiring intubation, suggesting a less severe course than previously thought.

Background:

  • Historically, it was considered a fulminant condition.

Purpose of the Study:

  • To characterize the clinical presentation and course of adult epiglottitis.
  • To evaluate the severity and management outcomes in adult epiglottitis patients.

  • Retrospective analysis of 28 adult epiglottitis cases.
  • Diagnosis confirmed via laryngoscopy and/or lateral cervical radiography.

  • Most patients had a benign clinical course.
  • Medical management (IV antibiotics, hydration, mist, corticosteroids) was effective.
  • Only 7% required orotracheal intubation; no respiratory arrest, obstruction, tracheostomies, or deaths occurred.

  • Prompt medical management is often sufficient for recovery.
  • The absence of Hemophilus influenza may correlate with a milder presentation.
