Summary

CD10 immunohistochemistry did not predict lymph node metastasis in colorectal cancer patients. Lymphovascular invasion alone was a better predictor of lymph node status than CD10 in this study.

Background:

  • Accurate preoperative prediction of lymph node status is crucial for effective colorectal cancer treatment.
  • The immunohistochemical marker CD10 has been proposed as a potential predictor of lymph node involvement in colorectal cancer.

Purpose of the Study:

  • To evaluate the association between CD10 expression and lymph node metastasis in a US patient cohort with colorectal cancer.
  • To assess the predictive capacity of CD10 for lymph node metastasis, both independently and in combination with lymphovascular invasion.

Main Methods:

  • Retrospective analysis of colorectal cancer specimens from 191 patients (stages T1N0-T3N2) at a single academic institution.
  • Clinicopathologic parameters, CD10 status, and lymphovascular invasion were assessed.
  • Statistical analysis, including multivariate analysis, was performed to determine predictors of lymph node metastasis.

Main Results:

  • CD10 showed a significant correlation with left-sided colon cancers and mucinous histology, with 75.7% specificity for lymph node metastasis.
  • CD10 did not correlate with lymph node status (p=0.33) and did not improve the predictive ability of lymphovascular invasion.
  • Lymphovascular invasion alone had a sensitivity of 62.8% and specificity of 93.6% for lymph node metastasis; adding CD10 reduced specificity to 69.3%.

Conclusions:

  • CD10 status was not significantly associated with lymph node metastasis in this patient population.
  • CD10 did not offer superior predictive value for lymph node status compared to lymphovascular invasion alone.
  • Multivariate analysis identified lymphovascular invasion as the sole significant predictor of lymph node metastasis.
