Background:

  • Eczema (atopic dermatitis) is a chronic inflammatory skin condition with significant impact on quality of life.
  • Understanding factors influencing eczema severity in adolescents is crucial for effective management and prognosis.

Purpose of the Study:

  • To identify factors associated with eczema severity in adolescents.
  • To analyze the progression of eczema severity from childhood to adolescence.

Main Methods:

  • Evaluated Nottingham Eczema Severity Score (NESS), atopy history, allergen sensitization (skin prick tests), serum IgE, and eosinophil counts.
  • Analyzed paired childhood and adolescent NESS scores in a cohort of 82 patients with a mean follow-up of 6.8 years.

Main Results:

  • Adolescent eczema severity correlated with infantile onset, sensitization to dust mites and food allergens, dietary avoidance, wet wrap use, traditional Chinese medicine, immunomodulators, high IgE, and eosinophilia.
  • Adolescent NESS was significantly associated with childhood NESS severity (P=0.034).
  • Of patients with paired scores, 48% maintained severity, 39% improved, and 13% worsened from childhood to adolescence.

Conclusions:

  • Eczema may not be outgrown in childhood, challenging parental expectations.
  • Long-term follow-up is essential for accurate eczema prognosis and management strategies.
