[Considerations on the "one and half" syndrome]

Two cases with a clinical disorder of extraocular movements characterized by a horizontal gaze palsy in one eye and failure of adduction in the contralateral eye are described. One patient had a hemorrhagic lesion in the pontine tegmentum giving the classic "one and half" syndrome first described by Fisher; the other a lung carcinoma metastatic to both cavernous sinuses. It is possible, even if not frequent, that peripheral lesions of the oculomotor nerves can manifest themselves with the same ophthalmoplegic symptomatology of central lesions.
