Summary

Glomerular CD44 expression in parietal epithelial cells (PECs) and visceral epithelial cells (VECs) is linked to segmental sclerosis in IgA nephropathy (IgAN). Increased CD44 in VECs indicates active glomerular injury and dysfunction.

Background:

  • CD44 is a marker for activated parietal epithelial cells (PECs) and is found in glomerular visceral epithelial cells (VECs) during segmental sclerosis development.
  • The study investigates the role of glomerular epithelial CD44 expression in IgA nephropathy (IgAN) with mild segmental sclerosis.

Purpose of the Study:

  • To explore the significance of glomerular epithelial CD44 expression in relation to segmental sclerosis in patients with mild IgA nephropathy (IgAN).

Main Methods:

  • 126 IgAN cases were classified into three groups based on glomerular morphology: normal, mild mesangial proliferation without sclerosis, and mild mesangial proliferation with sclerosis.
  • CD44-expressing parietal epithelial cells (PECs) and visceral epithelial cells (VECs) were quantified per glomerulus.

Main Results:

  • CD44 was expressed in VECs (59.5%), PECs (79.4%), and both (56.3%).
  • Higher CD44+ PECs or VECs were observed in IgAN with segmental sclerosis (group C) compared to groups A and B.
  • Increased CD44+ cells correlated with higher urine protein/creatinine ratio (UPCr).
  • Elevated CD44+ VECs were associated with increased UPCr, serum creatinine, and decreased estimated glomerular filtration rate (eGFR), even without segmental sclerosis.

Conclusions:

  • CD44 expression in PECs and VECs is associated with segmental sclerosis in IgAN.
  • Upregulated CD44 in VECs signifies active glomerular injury and dysfunction in IgAN patients.
