Primary congenital bladder diverticulum (PCBD) is a rare condition where the bladder wall is weak. The extravesical approach with diverticulectomy and ureteroneocystostomy is recommended for treating PCBD.

Background:

  • Primary congenital bladder diverticulum (PCBD) results from a deficient detrusor muscle layer, leading to bladder mucosa outpouching.
  • This condition can cause significant urological issues in children.

Purpose of the Study:

  • To review institutional experience with symptomatic PCBD.
  • To correlate clinical presentation with anatomical findings.
  • To evaluate the long-term outcomes of surgical management.

Main Methods:

  • Retrospective review of patients operated for PCBD since 2004.
  • Evaluation of clinical complaints, imaging, treatment methods, complications, and follow-up duration.
  • Surgical techniques included extravesical psoas-hitch ureteroneocystostomy and diverticulectomy, or intravesical treatment.

Main Results:

  • Ten patients (11 renal units) were treated, predominantly males, with a mean age of 5.3 years at surgery.
  • Common presentations included antenatal hydronephrosis and febrile urinary tract infections.
  • The ureter was implanted within the diverticulum in 72.7% of cases (8/11 RU).

Conclusions:

  • PCBD is uncommon, with a high incidence of ureteral implantation within the diverticulum.
  • The extravesical approach combined with diverticulectomy and ureteroneocystostomy is the preferred surgical technique.
  • The study demonstrated favorable outcomes with no reported complications during a mean follow-up of 34.1 months.
